The following form must be signed by your supervising professor:
- Download generic version (PDF, 2.3 MB), or
- personalised version (download as a part of the application process)

- personalised version (download as a part of the application process)
You must upload a complete official transcript of records in English or German of all your studies to date, i.e. including your Bachelor's degree studies if you are already a Master's degree student. The transcript should contain a grading key / legend explaining the grading system of your home university.
You must upload a scan of your valid passport: page(s) with your photo and personal details (name, date of birth) only. EU/EFTA citizens may upload a scan of their identity card instead.
If you have a second citizenship, please also upload proof of this.
You must upload a current curriculum vitae (CV) in English.
You must upload a photo of yourself in portrait format, which will be used for your ETH student card.
To note when choosing a photo:
- Recently taken
- Good quality image: full-face, well-lit, neutral background, etc.
- Maximum of 800 x 600 pixels (height x width) and 1 MB
